Insurance Coverage at Vital Nutrition
Yes—we accept insurance!
You may be wondering if your health insurance covers visits with a Dietitian on our team. The great news: most insurance companies cover nutritional counseling, especially for disease management like diabetes, kidney disease, or weight-related conditions. However, Florida has unique rules for commercial insurance, so we strongly recommend verifying your coverage before booking.
Most of our clients pay $0 out of pocket, but if your insurance does not cover the visit, our self-pay rates are:
Initial Assessment: $200
Follow-up Appointment: $100

**Medicare Requires a Physician Referral
For Medicare, a referral from your doctor is required with a qualifying diagnosis:
Diabetes: E11 or greater
Chronic Kidney Disease (CKD): Stage 3A or greater (N18.3 or greater)
Fax referrals to: 352-352-9390
TAX ID: 93-246846 | NPI: 1477330603
Tip: Medicare and some commercial plans may also require pre-authorization before your visit. Check with your provider before scheduling.
How to Verify Your Insurance Coverage
It is your responsibility to confirm coverage before scheduling. While we do our best to verify benefits, you are responsible for any charges not covered by your insurance.
Steps to check coverage:
Call the customer service number on the back of your insurance card.
Ask: “Do I have coverage for nutritional counseling with a registered dietitian?”
If asked for CPT codes, provide:
Primary: 97802 (Initial Assessment) or 97803 (Follow-up Visit)
Alternate: 99401, 99402, 99403, 99404, or S9470
If asked for a diagnosis code (ICD-10), provide:
Primary: Z71.3 (Dietary Counseling)
Alternate: Z72.4
Conditions that often qualify for coverage:
Overweight or obesity
Pre-diabetes or diabetes
Hypertension
High cholesterol
CKD Stage 3+
Preventive vs. Disease Management: Visits for general healthy eating may not always be covered. Disease-specific nutrition counseling usually has higher coverage.
Important Notes:
Some diagnoses (e.g., IBS without overweight/obesity) may require a deductible, copay, or coinsurance.
Insurance plans may limit the number of covered visits per year (typically 1–3).
Coverage rules vary by plan; always confirm before booking.
